Patient Records & Complaints
Texas law requires health care providers to provide clear instructions for requesting health records and filing a complaint. This page explains those options.
Request your health records
- Call (469) 678-0023 and tell the team you would like to request your health records.
- Provide your full name, date of birth, and a phone number or email address where the office can reach you.
- Tell the office which records you need and whether you would like to receive them electronically, by mail, or pick them up.
- Complete any authorization or identity-verification step requested by the office. The team will explain any applicable copying or mailing charge and when your records will be ready.

Contact the chiropractic licensing authority
The Texas Board of Chiropractic Examiners regulates chiropractors in Texas. You may contact the Board about a chiropractor or submit a written complaint through its complaint portal.

File a consumer privacy complaint
- For a concern about how protected health information was collected, used, shared, or handled, visit the Texas Attorney General’s patient privacy information page.
- Choose the privacy complaint option and follow the online instructions. Have the practice name and contact information available.
- Do not include highly sensitive information, such as a Social Security number or payment-card number, in a complaint.
